METHADONE MAINTENANCE PROGRAM

Bi-Valley's outpatient methadone maintenance is a structured program, which provides medical treatment of the brain chemical imbalance which results from long-term opiate use. Opiate addiction may result from heroin (i.e. morphine) use, or from prescription opiate use (i.e. vicodin, codeine, or oxycontin), or from smoking either heroin or opium.  Chronic use of any of the opiates produces the same brain hormone problem.  Methadone normalizes this chemical imbalance, without causing euphoria or other impairments.  Methadone treatment is perfectly compatible with work, parenting, and driving a car if taken properly.  Treatment is individualized and may last from a period of months to years, allowing for the essential long-term changes in the recovering addict's lifestyle, environment, and employment situations.  Individual counseling and support groups are integral components for successful outcomes in our program. MEDICALLY SUPERVISED WITHDRAWAL PROGRAM (MSW)

The medically supervised withdrawal (MSW) program is a 21 day program where methadone is administered in decreasing doses over a three week period.  This program is appropriate for those seeking treatment for the first time or for those that feel confident short-term treatment can produce positive long-term results (those who have failed previous withdrawal attempts are better candidates for longer-term methadone). Individual counseling, physical exams, urine screening, education regarding addiction and communicable diseases, HIV pre- and post-test counseling, and TB screening are included in the MSW program.  Referral for post-withdrawal treatment and participation in support groups are emphasized.  Amino acids and other medications are also available to assist with withdrawal symptoms once you exit the program. PERINATAL PROGRAM

The perinatal program provides methadone maintenance during and after pregnancy for women addicted to opiates.  Withdrawal from opiates during pregnancy is difficult and potentially dangerous to both the mother and unborn child.  Methadone maintenance is the standard of care for opiate-addicted pregnant women, which helps medically stabilize both the mother and the fetus.  Coordination of treatment with an obstetrician is mandatory. Patients receive specialized counseling focused on pregnancy, parenting, nutrition, and breastfeeding. The goal of the program is to help our mothers deliver healthy children and to support mother-infant bonding and long-term family stability.  Continued postpartum methadone treatment supports and reduces risk of relapse during the stressful period of parenting a newborn.   (Top of Page)

SOUTHEAST ASIAN PROGRAM

The Southeast Asian program provides methadone maintenance treatment services to Southeast Asian patients who are addicted to opiates, most often in the form of smoked opium.  Bi-Valley employs counselors who speak several languages (Hmong, Mien, Laos, etc.) and are knowledgeable about the cultural background of the patients.  Bi-Valley uses a separate assessment tool that is specific to the Southeast Asian culture, which helps our counselors better understand the treatment issues our patients experience.  Bi-Valley also keeps an up to date Southeast Asian Resource Binder at all three of our clinic sites for referral purposes.  (Top of Page)

MEDICAL SERVICES

Bi-Valley is experienced in assessing medical problems common to addiction and provides diagnosis, limited treatment, and referrals for all patients enrolled at Bi-Valley.  Some of the most common addiction-related medical illnesses are hepatitis C, skin infections from needle use (abscesses), and HIV.  Bi-Valley can also assist with consultations to other physicians and/or medical facilities to coordinate care for patients with co-existing medical problems. PSYCHIATRIC CLINIC

Bi-Valley is one of the only methadone treatment programs in the United States to offer psychiatric services at no additional cost to patients enrolled in our maintenance program.  Bi-Valley psychiatrists are experienced in treating co-occurring disorders (those with both addiction and mental health problems).  Untreated psychiatric problems are the most common antecedent to opiate abuse. Recovery is enhanced and facilitated by treating co-occurring mental health problems such as depression or anxiety. At times, recovery is impossible without this psychiatric care. (Top of Page)

PROPOSITION 36 PROGRAM

Bi-Valley Medical Clinic, Inc. is one of four local Proposition 36 approved treatment programs to help treat the drug addicted population.  Bi-Valley is the only methadone maintenance treatment program that is approved to administer Prop 36 treatment in the greater Sacramento area.  The Sacramento Abuse and Crime Prevention Act of 2000 (Prop. 36) offers adults who are convicted of nonviolent drug possession offenses the opportunity to receive substance abuse treatment as an alternative to incarceration.  This legislation went into effect on July 1, 2001.  This act treated over 30,000 people in its first year and this was the first treatment opportunity for over 15,000 of those offenders.  This program lasts 12-18 months.  Patients are referred by court order.  Attendance in weekly 1:1 counseling sessions and relapse prevention groups are mandatory.  A multi-disciplinary team meets weekly to evaluate treatment effectiveness and the need for ancillary services.  (Top of Page)
